I am the parent or legal guardian of the aforementioned Aspirant who is a minor, and hereby consent the Aspirant’s participation in the meditation training. I confirm that the Aspirant is fit to participate in the meditation training and am not aware of any medical condition that may restrict such an activity. I understand that any inherent risks involved are my sole personal responsibility. I also understand that there are no guarantees of outcome for this training. I hereby release, discharge, and agree to save harmless, its affiliates, volunteers, trainers, and employees from any liability or claimed liability in connection with the aforementioned meditation training. THIS CONSENT AND RELEASE is intended to be of perpetual duration.
